Pathways to Recovery: Substance Use Disorders Treatment Program
Pathways is an outpatient substance use disorders treatment program, providing high-quality, individualized care to adults living with substance use disorders. At Pathways, we believe in a harm reduction, low barrier approach to substance use disorder care. We focus on the needs of the whole person and building long term relationships with our patients.
Services provided by the Pathways Program:
- Assessment and treatment for substance use disorders (SUD) including opioids, alcohol, sedatives, cannabis, cocaine and methamphetamine
- Individual and group counseling
- Referrals to higher levels of care when needed
- Assessment and treatment recommendations for medical or behavioral health conditions that may be affecting SUD care
- Medication management for substance use disorders:
- Medications for opioid use disorder including Buprenorphine, Sublocade®, Brixadi® and Naltrexone (Vivitrol®)
- Medications for alcohol use disorder
- Medications for other substance use disorders
- Smoking cessation counseling and medications for patients being treated for other substance use disorders
- Harm Reduction education and services
- Outpatient withdrawal management for alcohol and other substances when appropriate
- Comprehensive discharge planning

What Members Can Expect:
- A thorough review of your medical history
- Recommended laboratory tests, including hepatitis C and HIV screening
- Recommendations for follow-up care with your primary care provider and/or specialists
- A comprehensive discharge summary and ongoing care recommendations shared with your primary care provider, behavioral health providers and specialists
- Referrals to community resources and support services
Pathways Program Care Options
- In-person: lower level of the GHC-SCW Capitol Clinic (675 West Washington Avenue, Madison, WI 53703)
- The GHC-SCW Capitol Clinic has a convenient onsite lab and pharmacy
- Virtual visits (video) are available
- Pathways is open Monday through Friday 8 a.m. to 5 p.m.
Getting Started
- For Members: Members may directly contact the Pathways Program coordinators at (608) 441-3320 to request an appointment.
- For Providers: GHC-SCW Primary Care, Urgent Care and Behavioral Health providers can place an order for Pathways in Epic and a member of the Pathways staff will contact you to obtain more information and schedule an appointment.
