GHC-SCW believes as an organization that health care is a human right. Addressing the health of many makes us Better Together. As a part of this, we have recentered our work and focused on improving access to quality health care and supporting the health and well-being of our members and community.
Population Health Department
The mission of the Population Health Department is to strengthen access to quality health care, connect patients to services, support and information, build community-based partnerships and develop community health programs.
Population Health Department Includes:

- Adopt-A-School Program
- Community Giving and Relations
- Social Work Services
Language Services
The mission of GHC-SCW’s language services is to provide interpreter services in Spanish, Hmong and Laotian at in-person appointments, video and telephone visits. Most of the Spanish, Hmong and Laotian translation requests are also completed by Language Services staff.
Language Services staff help members navigate GHC-SCW services, connect with health care resources and access information in their preferred language.
Community Outreach and Social Determinants of Health
At GHC-SCW we are committed to focusing our work around the five social determinants of health. They are the non-medical factors that influence health outcomes. They are conditions in which people are born, grow, work, live and age and the wider set of forces and systems shaping the conditions of daily life.
The five social determinants of health are:
- Economic Stability
- Health Care and Quality
- Social and Community Context
- Neighborhood and Built Environment
- Education Access and Quality
By understanding these factors and their impact on our members’ health, we can help connect people with resources and services that support better health outcomes.
