Learn More About Your Health Conditions
Use this resource library to help you better understand common health conditions and how GHC-SCW can support your care. Each health condition page includes helpful information, care resources and guidance to help you feel more confident managing your health.
Health information can sometimes feel overwhelming. These resources are designed to make it easier to learn about symptoms, prevention, treatment options and when to connect with your care team. You can explore topics such as asthma and COPD, anxiety, COVID-19, chronic pain, depression, diabetes, flu, hypertension, measles, mpox, pregnancy and RSV.
For additional trusted public health information, you may also visit resources from the Centers for Disease Control and Prevention and the Wisconsin Department of Health Services. These organizations provide up-to-date information about disease prevention, vaccines, outbreaks and public health recommendations.
If you are a GHC-SCW member and do not see your condition listed, you can sign up for ManageWell through the Member-Owner Rewards program. ManageWell gives members access to Mayo Clinic’s health information library, wellness resources and tools to support healthy habits.
Need Help Finding the Right Resource?
Your primary care provider can help you understand your diagnosis, answer questions and connect you with additional GHC-SCW services when needed. Depending on your condition, your care team may recommend clinical health education, behavioral health support, pharmacy services, specialty care or other programs to help you manage your health.
