To access your 1095-B online, use the link below to login to your GHCMyChart account.

Click Insurance.

View your tax documents, including your 1095-B.
Form 1095-B is an informational tax form that shows an individual and their dependents had minimum essential health insurance coverage during the year. It is provided for your records and to report coverage to the IRS.
You can now access your 1095-B through your GHCMyChart account.
No, you will not automatically receive a paper copy. Members can quickly and conveniently access their 1095-B through MyChart at any time. A paper copy is available upon request.
