GHC Foundations IOP
GHC Foundations Intensive Outpatient Program (IOP) offers evidence-based, multi-disciplinary and multi-modal care for adults with behavioral health diagnoses. This program would benefit patients who are either stepping down from higher levels of care or could use more intensive services than outpatient therapy and medication management.
We accept referrals from psychiatrists, primary care providers, mental health professionals. You cannot refer yourself.

What to Expect in the Program
All individuals will complete a comprehensive assessment with a team of licensed behavioral health specialists, including psychiatrists, therapists, behavioral health nurses and a transitional coordinator. Program participants will spend the majority of time with peers in a group therapy setting, learning and practicing skills while exploring symptoms and experiences in a safe environment.
Our curriculum uses evidence-based therapies, including cognitive behavioral therapy (CBT) and dialectical behavior therapy (DBT). Every person in the program will meet with an individual therapist and a psychiatrist at least once weekly. Although all individual care is different, we anticipate the average length of participation in our program to be two to four weeks.
Introducing Foundations Virtual Intensive Outpatient Program
GHC-SCW is pleased to introduce our Foundations Virtual Intensive Outpatient Program (VIOP), which allows you to receive behavioral health care from the comfort of your own home. Our VIOP provides the same high-quality, evidence-based therapy as our in-person sessions, and brings the support you need directly to you.
Through a secure telehealth platform, adults with behavioral health diagnoses will participate in comprehensive group therapy led by our licensed behavioral health specialists.

Referral Submissions
We accept referrals from psychiatrists, primary care providers, mental health professionals. We do not accept self-referrals. This program is not intended for individuals seeking care primarily for a substance use disorder or an eating disorder. Individuals with moderate to high risk of suicidal behaviors or aggression and threatening behavior to others should consider a higher level of care. Individuals who use substances must be able to minimize use sufficiently to attend program daily and be sober during treatment hours.
Referral Submission Information: Referrals will be reviewed within three business days of submission. Determinations regarding admission will be communicated to referring provider by secure email to the address provided on this form. Referring providers are responsible for submitting initial prior authorization requests to the participant’s payor. Upon acceptance and confirmation of prior authorization, GHC Foundations IOP staff will contact the participant by the phone number listed on this form to schedule admission.
