Once you renew your plan during the Open Enrollment period ending December 15 for your January 1, 2026 coverage and pay your first month’s premium:
- You will receive your GHC-SCW Member ID card(s) and a New Member Packet with plan details, ways to get care, online tools and contact information
- Contact Member Services to select a new GHC-SCW Primary Care Provider (PCP). If you do not select a new PCP, we will automatically assign you a GHC-SCW PCP based on your location. You can change your PCP at any time.
- Register for GHCMyChart to manage appointments, message your care team, view claims and more.
