Provider Maps & Directories​

GHC-SCW updates its provider directories on a quarterly basis. For the most up-to-date provider and clinic information, please use our Find a Provider/Find a Clinic search tool located on the left side of the screen.

• HMO Network Map: View all the clinic locations in our HMO Network. ​
• Individual Coverage Map: View all the clinic locations available to Individual Plan members.

• HMO Provider Directory: HMO Providers that are available in Dane County

• HMO Select Plan Directory: HMO Providers available to you when you enroll in a GHC-SCW Select HMO Plan (Select will appear in the plan title. Medicare Select Plan members may also utilize this network, but are not restricted to it).

• HMO Regional Network Directory: HMO Providers that are available in Columbia, Grant, Iowa, Jefferson, Juneau, Lafayette, and Sauk Counties.
  

• PPO Provider Directory: A national network of providers available to members enrolled in a GHC-SCW PPO plan. Individual Plan and Medicare Select Plan members are not eligible for PPO Plan coverage.
