Service Fees for Care

To inquire about fees for service at our GHC-SCW clinics, contact GHC-SCW Member Services at (608) 828-4853 or (800) 605-4327. For fees for services at in-plan University Hospital and Clinics, please contact your primary care clinic directly. If you do not have any insurance, you will be responsible for the entire fees for services listed.

Please note: The amount you pay out-of-pocket will depend on the benefit plan in which you are enrolled. For more information on your benefit levels, please contact your insurance company. If your insurance is through GHC-SCW, please see your Benefits Summary or contact GHC-SCW Member Services.
